A National Ambulance Service whistleblower fears patients lives are at risk over an “unfit” 999 system.

Its staff have raised serious concerns about its emergency response system over the recent death of father-of-three Ian McCarthy.

Noteworthy’s investigation for The Journal shows his passing is one of a number of deaths raised.

Mr McCarthy is from Roscrea and lived five minutes from a local ambulance hub.
